Requirements
Kent County as well as Ionia, Montcalm, Mecosta, and Osceola. We serve other counties as needed. We require a professional recommendation for any equipment that is requested. If a consumer is not currently receiving therapy services, we have occupational therapists on staff who are able to assess consumers for their equipment needs. We charge a copay (based on a person's income and sliding scale) minimum $10 for our assessments and/or any adaptive equipment loans made to the consumer.
Lending Period
As Long As Needed. The consumer can choose to call us when they are no longer in need of the equipment and we will pick it up or they can return the equipment to us at our agency.
Equipment
Wheelchairs, bath chairs, transfer benches, canes, walkers, raised toilet seats, transport chairs, tub bars as well as a variety of smaller items for activities of daily living (visual aids, items for dressing, eating, cooking, etc.). This inventory changes daily/weekly/monthly depending on the need of the consumers we serve and the amount/type of equipment donations we receive.While we assess consumers of any age, we do not carry pediatric adaptive equipment in our loan closet. We may have funding for consumers 50 and over to purchase equipment (in all counties we serve) and we also receive senior millage funds to assist with purchasing equipment for consumers 60 and over who live in Kent County. We charge an extra co-pay for any costly adaptive equipment.

ESM-AgrAbility staff helps farm operations adjust to and manage disabling conditions, such as, arthritis, chronic back pain, joint injuries, amputations, spinal cord injuries, hearing loss and any other condition that may limit a farmer’s ability to work.
Information and referral services and farm-site assessments are offered at no cost to the farmer. The cost of new equipment or modifications made to old equipment, the farm operation, or the farm home is typically covered through the state via Michigan Rehabilitation Services. ESM-AgrAbility staff will help the farmer navigate the state system to access these supporting funds.
